Official Rules: Onnit Total Human Customer Feedback Survey November 2025
TO ENTER OR WIN. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING.
Eligibility: The Onnit Customer Survey (the “Survey”) is open only to legal residents of the fifty (50) United States and the District of Columbia, who are 18 years of age or older at the time of entry. Void where prohibited or restricted by law. Employees of Onnit, its affiliates, subsidiaries, advertising and promotion agencies, and suppliers, (collectively the "Employees"), and immediate family members and/or those living in the same household of Employees are not eligible to participate in the Survey. The Survey is subject to all applicable federal, state, and local laws and regulations.
Agreement to Rules: By participating, the Entrant ("You") agree to be fully unconditionally bound by these Rules, and You represent and warrant that You meet the eligibility requirements. In addition, You agree to accept the decisions of Onnit as final and binding as it relates to the content of this Survey.
Survey Period: Entries will be accepted online starting on Thursday November 12, 2025 at 9am Eastern Time. All online entries must be received by November 20, 2025 at 12:00 am Eastern Time.
How to Enter: The Survey must be entered by completing the online survey in the email sent from Onnit. The entry must fulfill all Survey requirements, as specified, to be eligible to win a prize. Entries that are incomplete or do not adhere to the rules or specifications may be disqualified at the sole discretion of Onnit. You may enter only once. You must provide the information requested. You may not enter more times than indicated by using multiple email addresses, identities, or devices in an attempt to circumvent the rules. If You use fraudulent methods or otherwise attempt to circumvent the rules, your submission may be removed from eligibility at the sole discretion of Onnit. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible or misdirected registrations, which will be disqualified.
Prizes: Three (3) winners (each, a “Winner”) will be selected at random. The odds of winning a prize will depend on the number of eligible entries received. Each Winner will receive one (1) Visa® e-gift card with an approximate retail value (“ARV”) of one hundred fifty dollars (US $150). The total ARV of all prizes is four hundred fifty dollars (US $450). No cash or other prize substitution shall be permitted except at the sole discretion of Onnit. The prize is non-transferable and no substitution, assignment, or transfer of the prize to others, or request for the cash equivalent, is permitted. Any and all prize-related expenses, including without limitation any and all federal, state, and/or local taxes, shall be the sole responsibility of each Winner. Winner may be required to complete and return an IRS W-9 form (i.e. Request for Taxpayer Identification Number and Certification).
Winner Selection and Notification: Winners will be selected by Onnit under the supervision of Onnit. Winners will be notified by email within seven (7) business days following the selection. The individual must follow the instructions withing 48 hours or other period stated in the email. The email will include instructions from Sponsor that the potential prize winner must complete before he or she can be declared a prize winner and eligible to claim a prize. In the event the potential prize winner fails to adhere to Sponsor’s instructions, the potential prize winner may be disqualified from the Sponsor and ineligible to claim a prize. Onnit shall have no liability for the winner’s failure to receive notices due to spam, junk e-mail, or other security settings or for the winner’s provision of incorrect or otherwise non-functioning contact information.
POTENTIAL WINNER IS SUBJECT TO VERIFICATION BY SPONSOR. SPONSOR’S DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE ADMINISTRATION, OPERATION, SELECTION OF THE WINNER AND OTHER MATTERS RELATED TO THE SURVEY.
Verification of Potential Winner: Potential winner may be required to complete and return an Affidavit of Eligibility, Release of Liability and Publicity Release (where permitted by law) (collectively, the “Affidavit”) by the date specified by Sponsor, or an alternate potential winner may be selected. In the event (a) potential winner cannot be reached for whatever reason after a reasonable effort has been exerted, or the potential winner notification or Affidavit is returned as undeliverable, (b) potential winner declines or cannot accept, receive or use the prize for any reason, (c) of noncompliance with these Official Rules or any of the aforesaid time periods, (d) potential winner is found to be ineligible to enter the Survey or receive the prize, or (e) potential winner fails to fulfill the Affidavit-related obligations, the potential winner shall be disqualified from the Survey and an alternate potential winner may be selected in a random drawing, from among the other eligible entries received, as described above. Sponsor reserves the right to modify the notification and Affidavit procedures in connection with the selection of alternate potential winner, if any.
Terms & Conditions: Onnit reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend the Survey should virus, bug, non-authorized human intervention, fraud, or other cause beyond Onnit control corrupt or affect the administration, security, fairness, or proper conduct of the Survey. In such a case, Onnit may select the Winner from all eligible entries received prior to and/or after (if appropriate) the action taken by Onnit. Onnit reserves the right, in its sole discretion, to disqualify any individual who tampers or attempts to tamper with the entry process or the operation of the Survey or violates these Rules. Onnit has the right, in its sole discretion, to maintain the integrity of the Survey, to void votes for any reason, including, but not limited to: multiple entries from the same user from different IP addresses; multiple entries from the same computer in excess of that allowed by Survey rules; or the use of bots, macros, scripts, or other technical means for entering. Any attempt by an entrant to deliberately damage any website or undermine the legitimate operation of the Survey may be a violation of criminal and civil laws. Should such attempt be made, Onnit reserves the right to seek damages to the fullest extent permitted by law.
Limitation of Liability: By entering, You agree to defend, indemnify, release and hold harmless Onnit and its subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers, and directors, and any other person and organization responsible for sponsoring, fulfilling, administering, advertising or promoting the Survey, and all of their respective past and present officers, directors, employees, agents and representatives (collectively, the “Released Parties”) from and against any and all claims, expenses, and liability, including but not limited to negligence and damages of any kind to persons and property, including but not limited to invasion of privacy (under appropriation, intrusion, public disclosure of private facts, false light in the public eye or other legal theory), defamation, slander, libel, violation of right of publicity, infringement of trademark, copyright or other intellectual property rights, property damage, or death or personal injury arising out of or relating to a participant’s entry, creation of an entry or submission of an entry, participation in the Survey, acceptance, possession, attendance at, defect in, delivery of, inability to use, use or misuse of prize (including any travel or activity related thereto) and/or the broadcast, exploitation or use of entry. Winner acknowledges that the prize is awarded as-is without warranty of any kind.
Entrants further understand and agree that all rights under Section 1542 of the Civil Code of California (“Section 1542”) and any similar law of any state or territory of the United States that may be applicable with respect to the foregoing release are hereby expressly and forever waived. Entrants acknowledge that Section 1542 provides that:
“A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS THAT THE CREDITOR OR RELEASING PARTY DOES NOT KNOW OR SUSPECT TO EXIST IN HIS OR HER FAVOR AT THE TIME OF EXECUTING THE RELEASE, WHICH, IF KNOWN BY HIM OR HER, WOULD HAVE MATERIALLY AFFECTED HIS OR HER SETTLEMENT WITH THE DEBTOR OR RELEASED PARTY.”
The releases hereunder are intended to apply to all claims not known or suspected to exist.
The Released Parties are not responsible for: (a) any incorrect or inaccurate information, whether caused by entrants, printing errors or by any of the equipment or programming associated with or utilized in the Survey; (b) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software; (c) unauthorized human intervention in any part of the entry process or the Survey; (d) technical or human error in the administration of the Survey or the processing of registrations; or (e) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the Survey or receipt or use or misuse of any prize. If for any reason an entrant’s registration is confirmed to have been erroneously deleted, lost, or otherwise destroyed or corrupted, entrant’s sole remedy is another entry in the Survey. No more than the stated number of each prize will be awarded.
Disputes: THIS SURVEY IS GOVERNED BY THE LAWS OF THE STATE OF NEW YORK WITHOUT RESPECT TO CONFLICT OF LAW DOCTRINES. As a condition of participating in this Survey, participant agrees that any and all disputes that cannot be resolved between the parties, and causes of action arising out of or connected with this Survey, shall be resolved individually, without resort to any form of class action, exclusively before a court located in New York having jurisdiction and that: (1) any and all disputes, claims, and causes of actions arising out of, or connected with the Sweepstakes, shall be resolved individually, without resort to any form of class action; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with participating, but in no event attorney’s fees; and (3) in any such dispute, under no circumstances shall participant be permitted to obtain awards for, and hereby waives all rights to, punitive, incidental, or consequential damages, including reasonable attorney’s fees, other than participant’s actual out-of-pocket expenses (i.e., costs associated with entering this Survey). Participant further waives all rights to have damages multiplied or increased. Entrants agree not to raise the defense of forum non convenience.
Publicity: Except where prohibited or restricted by law, winner’s acceptance of prize constitutes the winner’s agreement and consent for Sponsor and any of its designees to use and/or publish winner’s name, city and state of residence, photographs or other likenesses, pictures, portraits, video, voice, testimonials, biographical information (in whole or in part), and/or statements made by winner regarding the Survey or Sponsor, worldwide and in perpetuity for any and all purposes, including, but not limited to, advertising, trade and/or promotion on behalf of Sponsor, in any and all forms of media, now known or hereafter devised, including, but not limited to, print, TV, radio, electronic, cable, or World Wide Web, without further limitation, restriction, compensation, notice, review, or approval.
Results: To obtain the prize winner list, send a self-addressed, stamped envelope to: Onnit Labs, Inc. Requests must be received within 60 days of the end of the Survey Entry Period.
